CONSENT, RELEASE AND AUTHORIZATION

I consent to the participation of Music Together^® classes operated by Becca Myers, Michaela Segelstein, All Aboard Family Music, including but not limited to regular classes, demonstrations, trial classes, make-ups and any sing a longs. I understand that such programs have many different kinds of movements by their nature, and I assume all risk of injury arising from such activity, and accordingly I release, indemnify and agree to hold harmless All Aboard Family Music, the teachers, Music Together^® LLC, any of the locations where class is held and the owner and operator in which the applicable program is located, and their respective directors, officers, employees, sponsors, and staff from any claim, suit, demand, or action arising in connection with my child’s participation in programs operated by Becca Myers and All Aboard Family Music I further assume all responsibility for all transportation to and from such programs.
